Community Support Services

Middle Flint Behavioral HealthCare’s Community Support unit seeks to utilize the strength of the individual, staff and community to assist the chronically mentally or emotionally ill citizens (co-occurring substance abuse or developmentally disabled/mentally retarded) achieving and/or maintaining the highest level of functioning possible.

Community Support Team and Community Support Individual differ in intensity but are similar in function. Both enable citizens to begin recovery with skills and resources to live independently in the community.

Community Support provides:

Education and training on skills and resources for independent living

Assessments of individual needs

Identification of available resources

Referral to needed services

Monitoring and coordination of interagency services

Direct service delivery of needed non-medical services not available in community

Community Support Team (CST) is symptom oriented, skill and resource development.

 

The admission criteria to this program includes:

have been discharged from inpatient psychiatric treatment or detox two or more times in the past eighteen (18) months.

 

For consumers with a DSM IV diagnosis, moderate to severe symptoms, and have four (4) or more of the following conditions:

Two (2) or more hospitalizations in the past eighteen (18) months

History of inadequate follow through with elements of a treatment plan related to risk factors (including lack of follow through taking medications, following a crisis plan or maintaining housing.)

Intermittently medication refractory

Co-diagnosis of substance abuse (ASAM - any level of care)

Legal issue (conditional release for nonviolent offense; history of failures to show in court)

Homeless or at high risk of homelessness due to residential instability

Clinical evidence for suicidal gestures and/or ideations in past three (3) months.

Ongoing inappropriate public behavior in the community within the last three (3) months

Self-harm or threats of harm to others within the last year

Evidence of significant complications such as cognitive impairments, behavioral problems or medical problems

A lower level has been tried or considered and found inappropriate at this time

Community Support Individual (CSI) is skill, resource oriented and stability maintenance.

 

Admission Criteria for Community Support Individual:

Consumers must have a primary mental health and/or substance abuse diagnosis with either

co-occurring, and/or mental retardation

The specific impairment(s) to be addressed must be delineated.

The individual’s level of functioning does not prevent the provisions of services in an outpatient level of care.

Individuals may need assistance with developing or maintaining skill to develop, maintain, and/or enhance social supports or other community coping skills.

Individuals may need assistance with daily living skills including coordination to gain access to necessary rehabilitative and medical services.

The treatment orientation modality and goals must be specified and agreed upon by the consumer/family/caregiver.

A citizen must be a resident in the eight county service area and meet the above requirements.

Community Support

Community Support services are offered to consumers to assist with skill building, community reintegration and resource linking. There are two types of community support services offered - community support individual and community support team. Community Support Individual is a less intense service and is designed to build skills and provide monitoring. Community Support Team is a more intense service offered by a team of service providers and is more symptom oriented. Services are frequently provided several times a week and are often provided in the consumer’s home.

Family Support Services

Families with members who are developmentally disabled or mentally retarded undergo a set of unique stresses and strains. Those family members often have additional physical problems compounding the family’s stress. Middle Flint Behavioral HealthCare’s Family Support Services program serves families with mentally retarded children age four and above or adult family members who are developmentally disabled.  Services offered include:

    Respite care to offer temporary care when the family needs a break.

    Personal support

    Day habilitation

    Personal living benefits

    Dental services

    Medical care

    Specialized clothing

    Specialized diagnostic services

    Recreation or alternative activities

    Environmental modification

    Specialized equipment

    Therapeutic services

    Counseling

    Specialized nutrition

    Supplies

The Family Support Services Program seeks to strengthen the family’s capacity to provide care. The overall goal is to promote development of a family life as close to that experienced by families without members with disabilities. We believe this could prevent unnecessary out of home placement, and to allow families to return a family member with disabilities to the home from a non-home residential setting.

Families meeting the following criteria are eligible for the Family Support Services Program:

    The individual with disabilities is four years old or older and has formally been determined to be "mentally retarded".

    The eligible individual with disabilities wishes to continue, or return, to home care but requires assistance to do so.

    The family of the eligible individual with disabilities wants him or her to continue or return to home care but requires assistance to make this possible.

    The authorized goods or services for which the family or individual is eligible will be sufficient to support or assist the continuance of, or return to, home care.
