We provide a variety of assisted living arrangements for clients such as our Community Residential Services, Semi-Independent Living Apartments, Supervised Cluster-Site Apartments, Developmental Training Home, Personal Care Homes, Group Homes. You may contact any one of Residential Services Offices Listed on this page at the number given. Individuals without natural supports or who may be at risk for institutionalization or able to maintain residence in their communities through the residential supports and facilities provided through Middle Flint Behavioral HealthCare. Community Residential Services Many of Middle Flint Behavioral HealthCare’s consumers have special needs regarding their living arrangements. By assisting these consumers function in the least possible restrictive environment through supporting the fulfillment of their residential needs, their optimum safety, security, and quality of life are assured. Consumers served by this program include adult developmentally disabled, adult mentally or emotionally ill, and chronic substance abusers currently in treatment. Adult residents in the eight county service area enrolled in of the day service community rehabilitation or developmentally disabled programs are eligible for referral to Community Residential Services. Community Residential Services include the following programs: Semi-Independent Living Apartments Supervised Cluster-Site Apartments Developmental Training Homes Personal Care Homes Group Homes Each residential program has an overall goals of assisting individual consumers in a managed approach to reaching obtainable individualized goals. Individuals goals may include learning basis independent living skills, community integration and socialization. Consumers are separated by gender. back to top of page back to Services page Semi-Independent Living Apartments The semi-independent living apartments are intended for higher functioning consumers. The consumers must possess the majority of skills necessary to live semi-independently with little staff or community intervention. If there is a history of substance abuse, sobriety must be exhibited at least 90 days prior to admission. Developmentally disabled/mentally retarded consumers may be eligible for the developmentally disabled/mentally retarded community residential semi-independent living apartments. Program staff offers support in the consumer’s home as needed to assist with money management, shopping, laundry, supervision of self-administration of medication, crisis management and other needs. back to top of page back to Services page Supervised Cluster-Site Apartments Consumers with a minimum of self-care skills, but not ready for semi-independent living, can benefit from a higher level of occasional supervision available at a cluster living apartment. Staff is on site during peak hours of activity, providing support and assistance in improving each consumer’s level of functioning in order to foster the consumer’s maximum level of independence. Supports include supervision of self-administration of medication, crisis management, finance management, shopping, laundry, some transportation, etc. Consumers in supervised cluster site apartments must no longer require twenty four hour supervision, but still need staff support in order to achieve the highest level of independent functioning. They have to participate in a daily structured activity such as school, job placement, community-based rehabilitation, or day support. Consumers must not have exhibited any violent or dangerous behavior within the past twelve months. back to top of page back to Services page Developmental training homes are intended for consumers lacking minimum self-care skills who require twenty four hour supervision. Separate training homes have been established for chronically mentally ill consumers and for developmentally disabled/mentally retarded consumers. Each training home offers residents support in developing daily life skills needed to avoid institutionalization allowing them to remain in the community. Criteria for developmental training home residents include: Have a history of an unsuccessful less intensive residential placement. Have multiple psychiatric hospitalizations for consumers with mental illness. Participate in a daily structured activity. back to top of page back to Services page The purpose of this type of home is to provide a family-oriented home-like setting for the developmentally disabled/mentally retarded and mentally ill consumer. Some personal care homes are operated by private providers under an agreement with Middle Flint Behavioral HealthCare. Each home must have at least one responsible caregiver on duty at all times. Residents of a personal care home must participate in a daily structured activity such as school, job placement, community-based rehabilitation, or day services. back to top of page back to Services page Group homes prove a less home-like environment residential setting within a community setting. Separate group homes are maintained for developmentally disabled/mentally retarded and mentally ill consumers with the consumers being separated by gender. The goal of the group home program is to help those who have been institutionalized or who severely lack independent living skills to obtain those skills necessary to move to a less restrictive setting. Twenty four hour supervision is provided with at least two staff members on duty at all times. Services available include: Twenty four hour protective care and watchful oversight. Supervision and assistance as needed for personal care. Nutritious meals and snacks daily for each resident. Assistance in daily living activities, laundry, clothing maintenance, shopping, etc. Recreation and leisure activities. Transportation for medical appointments, activities, etc. Continuous assessment of needs and condition. Training, as appropriate, to achieve goals of self-care and the possibility of moving into other levels of residential placement back to top of page back to Services page
